This paperwork must be completed for students who have been accepted in the YK Class for the 2024-2025 school year. Please note the last section of the paperwork re: committees & family helper days is not required for students in this class.

Please ensure this paperwork is submitted and that your first installment of tuition has been paid by Wednesday, May 15.
